The requirement of documents listed below:

1:1 Duly filled MTA 30/2Offline application (Original/You have to collect it from the Respective Mission)

1:2 Current Driving License issued by Department of Motor Traffic

1:3 Medical Certificate (From respective country) + English translation

1:4 Certified copy of valid Passport

1:5 Certified copy of valid Visa

1:6 Certified copy of National Identity Card (It is not mandatory if the NIC number is mentioned in the passport)

1:7 Two black and white passport size photographs

1:8 Certified copy of the receipt of payment made to the Sri Lankan mission Euro 100+Euro 12 per each document for certification

1:9 Delay charge Euro 10 per year
